Clinical Lead
A brilliant new job opportunity has arisen for a committed Clinical Lead Nurse to deliver a new model of care to support the people that use services within the company based in the Bath, Somerset area
You will be working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in BaNES, will be responsible for co-ordinating the provision of specialist interventions to meet the urgent health care needs of people at risk of hospitalisation or those requiring a facilitated timely discharge from ED and supporting a more intensive level of care in the community
As the Clinical Lead Nurse your key responsibilities include:
• To provide transformational leadership to create a culture of collaborative working to deliver services and outcomes of the highest standard
• Act as a point of escalation and support point for the CCC MDT when dealing with people with complex needs; working with clinical leads across multiple organisations to resolve problems
• Develop, implement, review and update relevant standard operating procedures ensuring all members of the CCC understand their role and are able to effectively support delivery of the pathways
• To lead the multidisciplinary team in the CCC and hold them to account for delivering objectives and key performance indicators such as 2 hour crisis response for urgent care
• Ensure the service is always seeking continuous service improvement and developing to meet the needs of the public
The following skills and experience would be preferred and beneficial for the role:
• Demonstrable knowledge of community health settings and co-dependencies with key stakeholders
• Excellent communication skills, both verbal and written and ability to communicate with internal and external staff and agencies
• Ability to regularly deal with and synthesize complex information and situations requiring analysis, and interpretation of such information to support decision making
